Migraines are far more common among adult women and often starts at puberty with the beginning of menstruation when there is a dramatic shift in hormones. Experts say that any fluctuation in estrogen can trigger a migraine in sensitized women. Birth control and pregnancy can reduce migraines for some women, but can increase them for others. In a small subset of women, migraine attacks begin when they start using OC and occur during the "non-hormone week."

Migraines that occur in women around their menstrual cycle are called "menstrual migraines." If a woman has predominantly menstrual migraines, the outlook for them may improve with the onset of menopause, when estrogen levels begin to drop off permanently

In general:

  • Use of OC should be determined based on its impact on your migraine situation
  • Use the OC with the lowest dose of estrogen since progestin does not seem to factor into the migraine equation
  • The risk of stroke in women with migraines appears to increase with the use of OCs or in the presence of high blood pressure or smoking
